He was born March 24, 1945 in Brooklyn, New York to Joseph J. Kozak.

Mr. Kozak was journeyman machinist and a Marine.  Mr. Kozak served his county in Vietnam.  He was very proud to be a Husband and father as well. 

Survivors include is wife Margaret, his two brothers Mr. Jack Kozak (Akron) and Mr. Gordon Kozak (Toledo), two sons Joseph J. Kozak II (Michigan), Mr. Brian Kozak (Columbus), three daughters Deborah Kozak and Patricia Kozak (Wellington, FL), Suzanne Lawson (Columbus)and his step children. Nine Grandchildren and step grandchildren.  He precedes his first wife, Theresa, in death. 

There will be no visitation. The body has been cremated. Arrangements were handled by Adams Funeral Home in Sidney.

Memorial Contributions may be made to the American Diabetes Foundation in his memory.
